It is important to let us know if you are a carer.

What defines a ‘Carer’?

A carer is defined as, someone who, without payment, provides help and support to a partner, relative, friend or neighbour, who could not manage to stay at home without their help due to age, sickness or disability. It should also be remembered that a carer may be a young person who is particularly vulnerable.

The Person for whom you care may or may not be a patient of this practice. If the person for whom you care is a patient at the practice it is useful for us to know this in case we need to contact you in an emergency.

Sometimes when people are looking after someone they can get tired or frustrated and would like to know that there is someone outside the family who might understand and can help them. If you are an existing patient who is a carer you can identify yourself to any member of staff and you will be asked to complete a Carer Registration Form or you can download the form from our patient resources section.

Register as a Carer

Please let us know that you are a Carer when you register with us. Not only can we take greater care of you, but we also need to know who is supporting our patients. The information you give us will be added to the notes of the person you are caring for, so we can contact you quickly in an emergency or if we have any other concerns. Also if you have concerns we know we can talk to you, as the main carer, with their permission.

Chaperone Policy

The surgery is committed to providing a safe, comfortable environment. Therefore we provide a chaperone for patients who require one whilst being examined by the Doctor.

Non NHS Fees

Some services are not covered by the NHS and a charge will be made in line with the BMA scale of fees.  – These include medical examinations e.g: HGV Licence ~ Employment medicals ~ Driving insurance etc.  – As well as certain certificates e.g: ~ Private insurance claims ~ Private sick notes ~ Holiday cancellations ...

Data Protection Act 1998

The Practice fully complies with the Data Protection Act 1998. Under the Act patients have a right to be informed whether personal data about them is being processed and have the right of access to the data.
